Job Description Job Description Benefits: 401(k) 401(k) matching Dental insurance Health insurance Paid time off Training & development Vision insurance Wellness resources JOB TITLE: Transportation and SCN Specialist Department: Social Care Navigation REPORTS TO: SCN Program Manager Responsibilities: The Transportation and SCN Specialist will operate within the framework of the Social Care Network, providing direct assistance to individuals to address the Health-Related Social Need of transportation. This role will primarily facilitate access to transportation with the agency van, and other community resources as time allows. The Transportation and SCN Specialist collaborates closely with the program manager and team members to best serve clients through transportation, screenings, assessments. Responsibilities will include but may not be limited to orientation & training provided by M&BPN and the Care Compass Network. Essential Job Functions: Transport clients to and from HRSN Related Services & Activities, assisting with mobility in and out of the van as needed. Ability to communicate clearly with clients about transportation guidelines, and provide a comfortable ride experience. To accurately obtain and record and enter data into MBPN and other systems. Maintain accurate driving logs, case notes, and other relevant documentation in compliance with SCN requirements and State & Federal guidelines. Collect and maintain all required records, reports and require statistical or other data. Assess a clients initial eligibility for Enhanced HRSN services following the SCN Screening that confirmed unmet Social Needs. Conduct interim eligibility assessments to determine if a client has experienced a status change that results in change of eligibility for Enhanced HRSN or other services and inform the SCN Lead Entity and HRSN Service Providers of any needed changes to the clients service eligibility. (Ex, the needs to end services or change end of service delivery dates when a clients Medicaid enrollment status changes.) Refer eligible clients to appropriate Enhanced HRSN to meet their needs. Assist Client who are at the end of their services delivery to ensure their need Enhanced HRSN services are completed, identify persistent needs, and provide support into transitioning services and support. This will also include helping and or advocating for the client to navigate barriers. Complete client follow-up/progress notes on each encounter and ensure client records are up to date. Attend all mandatory agency and department training courses, meetings and other groups as needed. Comply with all MBPN policies and procedures, as well as applicable state, and federal laws, rules and regulations related to the Social Care Network and the 1115 Waiver. Minimum Qualifications: Education: Associate Degree in Social Work, Human Services or a related field and: 2 years experience in care management, social service needs. An equivalent combination of education and experience. Valid drivers license with a clean driving record Ability to operate mini van and follow navigation instructions Ability to lift up to 25lbs and assist clients as needed Commitment to confidentiality and sensitivity when handling client information THE POSITION OF THE TRANSPORTATION AND SCN SPECIALIST IS A FULL-TIME, NON- EXEMPT POSITION, AND SUBJECT TO THE PROVISIONS OF THE FAIR LABOR STANDARDS ACT.
