Job Description Patterns Hiring is seeking a Social Work Skilled Nursing Facility (SNF) Licensed Clinical Social Worker for a travel job in Westfield, Pennsylvania. Job Description & Requirements - Specialty: Licensed Clinical Social Worker - Discipline: Social Work - Start Date: 03/23/2026 - Duration: 12 weeks - 40 hours per week - Shift: 8 hours, days - Employment Type: Travel Position: License Clinical Social Worker Substance Use and Behavioral Health Schedule: Tuesday–Saturday or Sunday–Thursday (one weekend shift required) Hours: 8:30 AM – 5:00 PM About the Role: The Counselor I will support clients in their recovery journey from substance use disorders, behavioral challenges, and co-occurring issues. The role involves conducting assessments, facilitating group and family sessions, supporting treatment planning, and providing crisis intervention. Key Responsibilities: - Conduct client assessments and develop individualized treatment plans. - Facilitate group and family therapy sessions. - Document client progress and update care plans accordingly. - Provide crisis intervention and assist with aftercare planning. - Collaborate with referral sources, insurance providers, and care teams. - Deliver education to clients, families, and the community on recovery and health topics. Qualifications: - Bachelor’s degree in social or health services (required). - Counseling internship/practicum experience (required). - Acceptable Licenses: LCSW, LPC, or equivalent, with experience in addiction recovery. - Preferred: PA state licensure, but out-of-state licenses will be considered.
