Job Description:
Community Options, Inc. is a national non-profit agency
providing services to individuals with disabilities in 11 states.
We are seeking a Full-Time TBI & NHTD Waiver Service Coordinator in
Binghamton, NY. The TBI Service Coordinator is an individually
designed intervention which provides primary assistance to the
Waiver Applicant/Participant in gaining access to needed waiver and
Medicaid State Plan Services, as well as other local, state and
federally funded services- medical, vocational, educational and
social. These interventions are expected to result in assuring the
Waiver Participants health and welfare and increasing independence,
community integration and productivity.Starting pay is
$19.00-$21.00/ hour, depending on
experienceResponsibilitiesMonitors fiscal, medical, personal, and
RSP & ISR documentation to ensure accuracyMonitors and ensures
accurate documentationMonitors and reports health and medical needs
of individualsWrites detailed plans and goal documentation for
service coordination Builds a support network between the
individuals and community membersCompletes monthly summary
documentation on each consumer in service coordination Ensures
consistent service delivery, which connects work, home and
recreational needsServes as an active member of each person's
Treatment Team including involvement in Individual Service Plan
(ISP) developmentSubmits monthly written reports covering
operational status, as well as individual progress and concerns to
supervisorCompletes weekly and bi-weekly billing and ensures
accuracy as based on consumer approved hoursCooperate with audits,
inspections, and investigationsKnowledge and SkillsAbility to
establish a comfortable and supportive relationship with a person
receiving supports.Flexibility to deal with people at a variety of
levels, both internally and externally.Excellent interpersonal and
communication skills.Excellent crisis and time management
skills.Ability to present facts and recommendations effectively in
oral and written form.Able to meet deadlines to ensure participants
continued eligibility in the waiver program.Minimum
RequirementsMaster's degree (Licensed SW, Clinical SW, Doctorate or
Master of Psychology, Gerontology, PT, Registered Professional
Nurse, Certified Teacher of Students with Disabilities, Certified
Rehab Counselor, Licensed Speech Pathologist, Occupational
Therapist).Minimum one year of experience providing case management
experience/ or Service Coordination and information, linkages and
referrals regarding community-based services for individuals with
disabilities and or seniorsBachelor's degree with minimum three
years one year of experience providing case management experience/
or Service Coordination and information, linkages and referrals
regarding community-based services for individuals with
disabilities and or seniorsAssociates Degree, with 5 years of
experience providing case management experience/ r Service
Coordination and information, linkages and referrals regarding
community-based services for individuals with disabilities and or
seniorsExperience completing functional assessments and work with
TBI preferredFirst Aid, CPR, OSHA, and Medication Certification
course within first 90 days of employment, and any required
residential or state trainingKnowledge and understanding of local
regulatory agency operations and Department of Health
RegulationsUnderstanding and commitment to community-based support
for persons with Traumatic Brain InjuryFlexibility to deal with
people at a variety of levels, both internally and
externallyAbility to present facts and recommendations effectively
in oral and written formExperience with problem solving against
multiple prioritiesAbility to exercise good judgment and remain
calm in crisis situationsExcellent interpersonal and communication
skillsExcellent time management skillsWorking ConditionsHours and
weekly schedule dictated by current tasks to include evening and
weekend hours as requiredOn call 24 hours a day in cooperation with
other management staffFrequent lifting, stretching, and other

job, employee travels by automobile, and is exposed to changing
weather conditionsMay be required to transport individuals
utilizing your own vehicle or company provided vehiclesMay be
