Family Peer Advocate
OVERVIEW:
Provide resources, education and support in a strengths-based,
individualized care approach to parents/guardians with a child with
a mental health diagnosis.
QUALIFICATIONS:
- - - - - - - - -High School diploma, GED or equivalent.
- - - - - - - - -Must have or have raised a child (including foster
child) with special emotional, behavioral or mental health
needs
- - - - - - - - -NYS Peer Advocate Credential - or the
ability/willingness to become one within 6 months of hire
- - - - - - - - -Family, Needs & Strengths certification- or the
ability/willingness to become certified
- - - - - - - - -Ability to work cooperatively with various service
providers in the community
- - - - - - - - -Ability to document activities, and comfortable
doing so in an electronic health record
- - - - - - - - -Ability to travel as needed
- - - - - - - - -Understanding of HIPAA/HITECH regulations
-- -Valid NYS Driver's License with a driving record acceptable to
MHAST's insurance carrier
-- -Must complete a criminal background check and a NYS Central
Registry Database Check in a manner acceptable to the NYS Justice -
- - -Center and Office of Children and Family Services
-- -Must not be on the NYS Medicaid Exclusion List
RESPONSIBILITIES:
The essential functions include, but are not limited to the
following:
- -Reduce use of costly mental health services through early
prevention, education, advocacy and supportive peer counseling
- - - - - - - - -Provide education and support to families as they
navigate the mental health and education systems
- - - - - - - - -Access and refer to appropriate community
resources
- - - - - - - - -Empower parents to be the decision makers in their
child's life by advocating for their child in the school and
community settings
- - - - - - - - -Accompany parents to meetings and appointments
when necessary
- - - - - - - - -Organize and facilitate family support group
- - - - - - - - -Create and maintain appropriate records including,
needs assessments, care plans, safety plans, progress notes,
utilization reviews and other pertinent documentation using an
electronic medical record
- - - - - - - - -Participate in regular case reviews with staff and
supervisor
- - - - - - - - -Collaborate with Single Point of Access (SPOA) and
other community providers
- - - - - - - - -Provide monthly statistics to Children & Family
Services Director
- - - - - - - - -Keep all case information confidential as directed
per agency policy
- - - - - - - - -Work collaboratively with MHAST staff
Individuals in this role will have access to Protected Health
Information (PHI) as part of their job duties:
Access is only allowed on a "need to know" basis to perform the
necessary job duties in this role
Access to the PHI of individuals being served outside of this
program is restricted (via the electronic medical record
system)
Access to, use of, and disclosure of PHI is to be done in
compliance with the agency's HIPAA policy manual
Nothing in this job description restricts management's right to
assign or reassign duties and responsibilities to this job at any
time
Reports to: Director of Children and Family Services
Status: Non-exempt
Hours: Full time
