JOBDESCRIPTION Title Specialist - Medical Advertising Title Nurse Evaluator Grade Position Classification Exempt Division Health Services East Department Independent Entity Reports To Manager - Medical Required Education Graduate of an accredited nursing program Preferred Education BSN in Nursing and/or equivalent certification Required Licensure Current RN Licensure in New York State Required Experience 2 or more years' experience providing home care Preferred Experience Home care experience using the UAS-CANS-NY system; RN experience in a psychiatric / behavioral health / pediatrics / foster care settings / group homes / etc. Required Skills Ability to review health status questions and make evaluations and determinations based on the UAS-CANS-NY system criteria for enrollment; ability to work with technology in the workplace;-- knowledge of the theory, principles, and practices of general professional nursing; knowledge of state and agency laws and regulations governing general professional nursing practices; excellent organizational, interpersonal, written, and verbal communication skills; ability to communicate and work effectively with multi-generational consumers; ability to demonstrate sensitivity to the concerns of others; ability to perform comfortably in a fast-paced, deadline-oriented work environment; ability to successfully execute many complex tasks simultaneously; and a strong ability to work independently Preferred Skills Knowledge of motivational interviewing techniques Bilingual May be required in commonly spoken languages such as Spanish, Russian, Haitian - Creole, Chinese Other Travel required; NY State Driver's license required; vehicle may be required depending on the region of coverage JOB RESPONSIBILITIES1.--------Develop Home and Community Based Services(HCBS) Plan of Care (POC) in collaboration with child and family that includesevaluation of health status2.--------Perform initial in-home evaluations ofchildren that includes evaluation of health status, strengths, care needs, andpreferences and guide the development of individualized long-term care serviceplans3.--------Review child's medical documentationand or health referral forms, as relevant to the case4.--------Utilize the State CANS assessment tooland MAXIMUS case management systems to document the child's health status,strengths, care needs, diagnoses and preferences and transmit, as required 5.--------Properly document any concerns,conflicting information, other issues that surface during the evaluationprocess 6.--------Emphasize continuity of care, thusreducing or eliminating fragmentation, duplication, and gaps in treatment plan 7.--------Discuss with child/family health careoptions, supports needed, service vendor options and waiver options8.--------Update plans of care as per Staterequirements, and on an as needed basis9.--------Collaborate with Family SupportCoordinators to meet the needs of the family10.----Enhance communication andcollaborative relationships with interdisciplinary care team members to improvecare coordination and facilitate service delivery11.----Collect quality review data and anyrequired documentation to support outcome measurements and record case notesinto CRM 12.----Provide relevant information inresponse to the child's specific needs as a bridge-- between when the in-home evaluation isconducted and evaluation results are provided13.----Maintain a comprehensive workingknowledge of community resources and network services for target population14.----Meets all standards established forthis position as outlined in the corresponding annual performance criteria andbonus template for this position15.----Performs other duties as may beassigned by the Regional Manager or other project managementBenefits At Employer, we firmly believe that our employees are the heartbeat of our organization and we are happy to offer the following benefits: Competitive pay & weekly paychecks Health, dental, vision, and life insurance 401(k) savings plan Awards and recognition programs-- *Benefit eligibility is dependent on employment status.-- Employer is a leading provider of home healthcare, medical-related staffing, travel nursing, and population health and wellness services across the United States.
