JOB TITLE Social Care Network Coordinator DEPARTMENT
Administration FLSA CLASSIFICATION Non-Exempt REPORTS TO Corporate
Compliance Officer DATE March 2026 JOB SUMMARY Responsible for the
screening, monitoring, and oversight of all Healthy Alliance Social
Care Network activities within the Rescue Mission of Utica.
ESSENTIAL JOB FUNCTIONS Conduct screenings and referrals of clients
through UniteUs system. Responsible for monitoring and tracking of
data related to Healthy Alliance services provided to clients
Generate and submit client data reports to supervisor Coordinate
incoming and outgoing referrals with program staff & supervisors
Responsible for gathering and processing client data and
documentation Conduct ongoing Healthy Alliance trainings with
approved staff and supervisors Stay abreast of all updates and
regulatory changes in the Medicaid Redesign Project on both federal
and state levels. Build trust and rapport with clients of all
Rescue Mission programs Abide by all Mission, program and safety
policies, procedures and guidelines. Attend and participate in
regular meetings and trainings as requested by your supervisor and
Healthy Alliance. All other duties as requested by your supervisor.
MINIMUM REQUIREMENTS High School Diploma or equivalent. Minimum 2
Years experience in Human Services required Associates degree in
related field preferred. NYS Driver’s License required Familiarity
with database usage and data entry. Experience with health
management software Advanced skills with Microsoft Word, Excel, and
Outlook. Enthusiastic support of our Mission Statement.
Status/Hours: Full time, Monday- Friday – 8am-4pm – 40 hours per
week Pay Rate: $25.00-27.00 per hour Benefits: We offer a
