Job Description:
Position Type/Expected Hours Of Work – 40 hours per week; based
in a transitional residential setting with regular interaction with
clients, staff, and community partners. The role may require some
evening or weekend work to meet program needs. Qualifications –
Bachelor’s Degree in Social Work, Human Services, or related filed,
Masters Preferred. Experience in case management in a mental
health, social services, or housing program. Experience working
with people with intellectual/developmental disabilities preferred.
Work Authorization/Security Clearance – Authorized to work in the
United States. Summary/Objective – Responsible for providing
comprehensive case management for the Critical Time Intervention
(CTI) program, a short-term intensive case management program which
provides support for individuals with dual diagnoses of mental
health conditions and developmental disabilities in a time of
transition. Support clients to gain and maintain resources and
stability in the community. Essential Functions – Reasonable
accommodations may be made to enable individuals with disabilities
to perform the essential functions. Implement program goals,
objectives and strategies in alignment with client needs and
relevant methodologies. Coordinate in person meetings with clients
and complete all assessments and collaboratively develop CTI phase
work plans with clients. Facilitate networking with community
mental health partners and other community organizations to
enhance, service delivery and support client needs. Educate and
work with families, caregivers, natural supports and community
partners to support client stabilization in community and at home.
Coordination of services, including mental health, substance use
treatment, housing, employment, social services, and any addition
services client may require. Advocate for clients, working to
reduce barriers to services and increase access to resources.
Monitor client progress and outcomes and adjust service plans as
needed to support successful transitions to permanent housing and
stabilization of mental health. Maintain accurate and up-to-date
client records, data, and documentation in accordance with
organizational policies and funding requirements. Deliver
personalized, culturally competent, and trauma-informed care to all
program recipients Attend weekly CTI team meetings. Competencies –

communicate with various socio-economic and culturally diverse
backgrounds Good organizational and time management skills
Understand and embrace person-centered services Computer literacy
Travel – This job may require travel to complete assessments,
attend meetings or conferences. Work Environment – This job
operates in a specialized residential setting with designated
office space for management and clinical staff. This role routinely
uses standard office equipment such as computers, phones,
photocopiers, filing cabinets and fax machines Physical Demands –
