Job Description:
Job Description The Professional Staff Nurse/Case Manager at
UPMC Muncy Place is responsible for delivering comprehensive
nursing care and coordinating with interdisciplinary teams,
residents, families, and external partners to ensure high-quality
outcomes and seamless transitions post-discharge. This role
involves managing both short- and long-term residents, supporting
care planning, leading Crisis Intervention training, and promoting
wellness through chronic condition management and education. The
Case Manager collaborates with the Quality Improvement Team to
monitor and reduce Emergency Department visits and
hospitalizations, identifies educational needs, and provides
guidance to frontline staff—all while applying age-appropriate care
principles and maintaining a resident-centered approach. /n /n The
ideal candidate will have the following: 2 years of RN experience,
5 years working in a Long Term Care facility, and a BSN. /n /n
Schedule: Full-time, 40 hours/week (Day Shift, Monday–Friday) /n
Holiday Rotation: 1-2 holiday shifts per year Weekend Rotation:
Minimum 1 weekend shift every 4 weeks /n /nSign-On Bonus/n /n -
$15,000 for new graduates (2-year commitment) /n - $20,000 for
experienced nurses (2-year commitment) /n /n /nKey
Responsibilities/nResident Care & Case Management/n /n - Manage
care for short-term and long-term residents /n - Collaborate with
physicians and care teams to ensure quality care /n - Follow
residents with chronic conditions and promote overall wellbeing /n
- Assist with care planning and discharge preparation /n - Provide
psychosocial support and counseling /n - Monitor and document
changes in resident condition or behavior /n /nQuality & Safety/n
/n - Partner with the Quality Improvement team to reduce ED visits
and hospitalizations /n - Comply with medication safety and
infection control policies /n - Identify opportunities for clinical
improvement and innovation /n /nEducation & Development/n /n - Lead

education needs /n - Provide feedback and education to frontline
staff /n - Engage in continuous learning and professional
development /n - Participate in RN residency programs and seek

Qualifications/n /n - Long-Term Care Experience: Preferred /n -
Education: Bachelor of Science in Nursing (BSN) preferred /n
/nLicensure, Certifications, and Clearances/n /n - Basic Life
Support (BLS) or Cardiopulmonary Resuscitation (CPR) certification
required within 30 days of hire (must meet AHA standards including
didactic and skills demonstration components) /n - Registered Nurse
(RN) license or Temporary Practice Permit (TPP) required /n -
Licensure Requirements:/n /n - Must hold current licensure in the
state where the facility is located /n - If licensed under the
Nursing Licensure Compact (NLC), a multistate license from a
participating NLC state is acceptable /n - Employees who relocate
