An LPN works under the direction of a registered nurse and is responsible for handling a diverse range of tasks, including administering treatment or medications, performing clinical procedures, documenting clinical data of patients, and more.RESPONSIBILITIES:Ensures the care and safety of all residents in the nursing unitEnsures that individual care plans are followedAdministers and documents medications, treatments, and other nursing servicesMakes a count of controlled substances on the assigned nursing unitRecords current condition of assigned resident in EMRReceives and gives an accurate report of the resident condition at shift changeCommunicates to the nursing team and medical provider as indicated and requiredAssists in the orientation of new staffMaintains competency in all the tasks needed by CMSREQUIREMENTS:Minimum 18 years of ageNew York State Licensed Practical Nurse licensureStrong written and verbal communication skills; able to follow written and verbal instructionsBasic computer knowledge, PCC e-MAR experience preferredPhysical endurance for frequent activity; ability to lift a minimum of 20 poundsABOUT US:VestraCare is a portfolio of four health care facilities in New York state, including: Chautauqua Nursing and Rehabilitation Center in Dunkirk; Roscoe Rehabilitation and Nursing Center in Roscoe; Sunset Nursing and Rehabilitation Center in Boonville; and Susquehanna Nursing and Rehabilitation Center in Johnson City.
