Assessment Nurse at United Methodist Homes?
- Licensed as a Registered Professional Nurse in the state of New
York.
- Current knowledge and practice of accepted standards of
professional nursing practice.
- Working knowledge of pharmacology.
- Positive interpersonal skills in working with residents,
families, and interdisciplinary team members.
- Ability to plan, develop, implement, and interpret the
programs, policies, and procedures for quality care.
- Subject to call-back during emergency conditions, i.e. severe
weather, evacuation, disaster, etc.What will I do in this role as
an RN Assessment Nurse at United Methodist Homes?
- Perform assessments in relationship to change in resident
status.
- Complete the nursing assessment, pressure ulcer risk
assessment, fall risk assessment, pain assessment, customary
routine, physical restraint determination, and the elopement risk
assessment at intervals defined by United Methodist Homes'
policy.
- Identify and document actual and potential resident problems,
symptoms, and changes in condition, and take appropriate
action.
- Assist in the care plan process in conjunction with the IDT to
ensure appropriate treatments and /or interventions are implemented
according to resident needs and physician orders
- Provide leadership and supervision to nursing personnel.
- Communicate effectively with others, i.e. residents, families,
visitors, and co-workers.
- Have knowledge of and adhere to facility policies and
procedures.
- Identify the learning needs of residents/families and family
caregivers and help to develop and implement a teaching plan.
- Complete all necessary documentation in an informative and
descriptive manner that reflects the care provided to the resident,
as well as the resident's response to the care.
- Understand and follow all HIPAA Guidelines.
- Understand and follow infection control practices.
- Subject to call-back during emergency conditions, i.e. severe
