Successful candidates will possess a high level of critical thinking skills, be a proven performer in motivating team members and be an active, collaborative member of multi-disciplinary teams.Responsibilities include:--Ensures focus and activities that support clinical department metrics by ensuring timely and accurate documentation, clinician productivity at agency standard and ensures measures are cost effective; acts as a program liaison with the telemonitor vendor and provides oversight of the program; assists Clinical Educator in planning and conducting the agency's orientation program for new professional staff and ongoing education and competency needs.BSN and two years' home care experience as a community health nurse or a critical care nurse along with valid NYS RN license.Schedule - M-F, days and participation in on-call rotation.This area boasts beautiful rolling hills, a major university, lakes and rivers, parks and trails, a small-city environment, and an affordable housing market!
