Nurse Care Manager, UHS Wilson Medical CenterUnited Health Services is seeking a Nurse Care Manager to join our WMC Care Management department at UHS Wilson Medical Center in Johnson City, NY. This position is benefits eligible.ShiftEveningHours per week:28Salary range:$35.27 - $53.94 per hour, depending on experience.OverviewNavigates and coordinates the care of patients across the continuum of care. Monitors and manages resources to transition patients to the appropriate level of care in a cost-effective fashion while ensuring quality outcomes and patient satisfaction. Assesses, plans, coordinates, monitors and evaluates the patients plan of care in collaboration with the healthcare team. Monitors the patient care process and collects data to evaluate and improve care and collaborates with physicians and other health care workers to provide comprehensive care including health risk appraisal, health promotion and clinical outcome studies. Educator payers, providers and the public and serves as the information source regarding state and national health care trends; initiatives, which enhance the provision of, care and ensure efficient resource utilization.QualificationsEducation/ExperienceMinimum Required:Associates in Applied Science in NursingMinimum of two (2) years of experience in acute/tertiary facilityPreferred:Bachelors of Science in NursingFive (5) years of experience in acute/tertiary facilityLicense/CertificationMinimum Required:NYS Registered NurseAbout United Health ServicesUnited Health Services is a not-for-profit healthcare system serving more than 500,000 people in Upstate New Yorks Southern Tier region.
